To authorize the Fund to securely deposit your monthly pension benefit into your chosen bank account (excluding brokerage accounts), complete the Direct Deposit Authorization form. You can request this form by calling participant Services at (212) 499-4800 or toll-free at (800) 562-4690 or by sending a message through the Portal's Help Center Page. Include your name, your email address and note “request for Direct Deposit Authorization” in the form as your reason for your inquiry.
A voided check, savings slip or bank confirmation is required with this form. All signatures on your form must be notarized by a Notary public in order for the Fund to process your Direct Deposit Authorization Form.
